Uniform Parenting Plan form
Pursuant to Public Chapter 127, amending T.C.A. §36-6-404, the Administrative Office of the Courts has developed a parenting plan form that shall be used consistently by each court within the state that approves parenting plans pursuant to §36-6-403 or §36-6-404. The form is to be used on and after July 1, 2005.

Based on this new public chapter, the AOC, in consultation with the Family Law Section of the Tennessee Bar Association and the Tennessee Judicial Conference Domestic Relations Committee and other knowledgeable persons, finalized the form.

The Microsoft Word fill-in form should be used as follows:

1. Open the Microsoft Word fill-in document. You will be automatically positioned at the first entry field.

2. Enter information in the field. Press tab to advance to the next field.

3. Continue as in step 2 for the remainder of the fields. You may press the space bar to place an X in a checkbox field. If you have placed an X in error in a box, pressing the spacebar again while you are in the field will clear the X. To return to a previous field, press Shift+tab. (Tab moves forward field by field, shift+tabmoves backwards). Check boxes should be left blank as you “tab through” those that do not apply.

If the text entry field does not apply to the current order, please enter a space before tabbing to the next field. If you do not press the spacebar, or do not make an entry, the help text will continue to show in the document for that field.

4. When you have completely finished all entries and the focus has returned to the beginning of the document, choose Save As and specify a filename and location.

5. If you wish to edit the form, you must choose Tools, Unprotect Document.

6. If you have edited the document you must resave.

7. Print the document.
